     If you are interested in adopting one of our puppies (even if there's not a litter available right now), we would appreciate your taking the time to fill out the following form.  We want to make sure our puppies find good homes.  Please answer each question honestly.  If you have never owned a dog, or don't have a vet, that's ok.  It's not necessary to have owned a dog or to have a vet to adopt one of our puppies, but for those who have, it can assure us that we are adopting our dogs to good homes.  Thank you.
